Re(1): Stealing the serve in singles

A while back, I had asked about positioning of the server and wondered how far left he could stand. How easy/difficult is it for a server to put enough 'reverse english' on the serve to put it above the cage? If it's easy, I could see why a players' manager would frown upon this (no volleys), but if it's difficult, then why would it be frowned upon?

Can any former players expound on this?
